A turning point in oncology

Radiopharmaceuticals are not new. Therapies like radioactive iodine have been used for decades. However, the field is now at an inflection point, driven by the rise of theranostics—a precision medicine approach that combines targeted diagnostics with targeted therapy.

Several factors are fueling this resurgence:

  • A growing number of biological targets (e.g., PSMA, SSTR2, FAP)
  • Advances in isotope production and supply chain capabilities
  • Expansion into new indications, including earlier lines of therapy and combination approaches

Together, these advances are accelerating clinical development and positioning radiopharmaceuticals as a key modality across multiple solid tumors.

Beyond a drug: A complex modality

Radiopharmaceuticals are unique in that they combine a targeting molecule with a radioactive payload, enabling highly precise delivery of radiation to tumors while minimizing exposure to healthy tissue. This dual nature makes them powerful, but also introduces significant complexity, particularly in clinical trials.
Unlike traditional oncology studies, radiopharmaceutical trials require:

  • Specialized infrastructure (nuclear medicine, radiopharmacy)
  • Cross-functional collaboration across departments
  • Highly trained staff familiar with radiation handling and safety

This creates an additional layer of coordination that many sites are not yet fully equipped to manage.

The critical role of logistics

If there is one defining feature of radiopharmaceutical trials, it is the importance of timing. Many of these therapies rely on isotopes with short half-lives, meaning production, delivery and administration must be precisely coordinated.

There is no “off-the-shelf” model, manufacturing often begins only once a patient is scheduled.
Any disruption, whether in production, quality control or transport, can result in missed treatment opportunities.

This makes radiopharmaceutical trials among the most operationally intensive in clinical research, requiring seamless coordination across sponsors, CROs, sites, manufacturers and logistics providers.

Operational complexity at every step

From regulatory requirements to site readiness, radiopharmaceutical trials demand a more rigorous and integrated approach. Key considerations include:

  • Country-specific radiation protection regulations
  • Stringent quality control from production to administration
  • Imaging standardization across sites
  • Compliance with companion diagnostic regulations

These requirements can extend study timelines and limit participation to highly specialized centers, though efforts are underway to expand capabilities more broadly.

Keeping the patient at the center

Amid this complexity, the patient experience remains paramount. Scheduling is not just about clinic availability, it depends on a tightly aligned chain of events, from isotope production to staff readiness.

When disruptions occur, the impact goes beyond timelines and costs. Delays or cancellations can affect patient confidence, site morale and overall trial engagement highlighting the need for reliability and precision at every step.

Innovation requires a new mindset

Radiopharmaceuticals represent more than a scientific breakthrough, they demand operational excellence. Success in this space depends on understanding the full ecosystem and ensuring every component works in sync.
